First up, was a product recall from Pacific Cycle of their swing sets that were discovered to have a fall hazard. These swing sets were sold exlusively at Toys R Us.
The product recall involved the “Playsafe Dartmouth Swing Set” model number 22-PS340, with date codes FSD0115AA and FSD0315AA. The issue was that the seats can crack or split, which could then pose that fall hazard that they were concerned with.
They received five reports of minor injuries from the product.

The 2nd product recall on this list involves a climber tool. It’s called a “GRIGRI 2 belay device with assisted braking” and it was put out by Petzl America & Manufactured by “Petzl SAS, of Crolles, France.”
There have been almost twenty thousand (20,000) units sold, which makes for a lot of fall hazards.
The product recall does not have any injuries reported and involves the device called the “The GRIGRI 2 belay.”
A few users started noticing that with enough force applied to the handle, it could become stuck in the open position and that disables the assisted braking function. No braking, no stopping and technically, you are now falling!
The serial numbers of the specific items involved in this product recall start within this range: 10326 to 11136. Those are the first five numbers of the serial number.
You can contact Petzl America at (800) 932-2978 or hit up their webpage that addresses this recall at
